We are grateful that you trusted us. There are a few preparations to be made before you make your appointment.

1. You need to register after you reach the place. So we request you to reach here 30 minutes early. This will help you to check-in and then register your name.
2. Carry your insurance card or your ID for reference proof.
3. If you hold HMO plan then by using your insurance you can designate a doctor. The doctor will act as your PCP- which stands for primary care physicians. This helps and makes it easy for us to build the insurance for the service that you receive from our side and then we can refer you to any other specialist if needed. At the point of your visit co-payments remains due.
4. If you have any queries or questions regarding financial responsibility for the coverage then you are requested to give a call on the number mention at the back of the card. The queries must be done before your first visit because the insurance number is unique.
5. While making your first visit please carry the list of medicines which you are consuming already.
6. Carry the medical prescription of the previous doctor’s whom you have visited.
7. Carry a list of things which you want to discuss with the doctor or whomever you are visiting. Don’t hesitate to open up your problems in front of your provider.

The forms mentioned here can be downloaded by the patients. You are provided with instructions.

2. You need to open the file with the application- Adobe acrobat.
3. Take a printout of the form that you have filled up.
4. Don’t forget to bring this printed form of the application when you make a visit.
